PLEASE READ THESE TERMS AND CONDITIONS CAREFULLY. BY ACCESSING OR USING OUR ONLINE SERVICES, YOU AGREE THAT YOU HAVE READ, UNDERSTOOD, AND AGREE TO BE BOUND BY THESE TERMS AND CONDITIONS, AS AMENDED FROM TIME TO TIME.

THE ONLINE SERVICES ARE NOT INTENDED FOR USE IN THE EVENT OF A MEDICAL EMERGENCY.

If you do not agree to these Terms and Conditions, do not access or use any part of the Online Services.


1. Introduction & Acceptance

Arizona Psychiatry (“we”, “us”, or “our”) operates the website www.arizonapsychiatry.com (the “Online Services”), which provides information about and facilitates appointment requests for adult psychiatric services in Tucson, Arizona, USA. By accessing or using the Online Services, you agree to these Terms and Conditions and our Privacy Policy. We may update these Terms and Conditions at any time, and any amendments or updates are effective immediately upon notice—including by posting a revised version on our homepage.

We reserve the right to take whatever lawful actions we deem appropriate in response to actual or suspected violations of these Terms and Conditions.


2. Services and Purpose

  • Scope of Services:
    Arizona Psychiatry provides psychiatric services exclusively for adults within the United States. The Online Services are intended solely to provide information about our practice and to facilitate appointment requests.
  • Appointment Requests & Information Collection:
    When you request an appointment, we may collect personal information including your name, contact details, billing information, and protected health information (PHI) needed to schedule and provide treatment. Note that patients do not create user accounts, and all collected data is used solely for patient care and administrative purposes.

3. Medical Disclaimer & Emergency Use

  • No Medical Emergency Use:
    The Online Services are not intended for use in the event of a medical or psychiatric emergency. If you are experiencing an emergency, please call 911 immediately.
  • No Medical Advice/Patient-Physician Relationship:
    The information provided on the Online Services is for informational purposes only and is not intended as a substitute for professional medical advice, diagnosis, or treatment. An initial appointment (or “Intake”) is considered a consultation only and does not establish a patient-physician relationship. Always seek the advice of your health care provider regarding any medical or psychiatric concerns.

4. Intellectual Property

  • Ownership & Restrictions:
    All content on the Online Services—including text, images, logos, audio, video, and software—is the property of Arizona Psychiatry or its licensors and is protected by intellectual property laws. No right or license is granted, by implication or otherwise, to use any trademark, service mark, or logo displayed on the Online Services without our express written permission.

While the Online Services do not directly process payments, we do collect credit card information via a secure, encrypted DocuSign form to manage patient responsibilities such as copays, deductibles, and coinsurance. All payment-related data is handled securely and solely for the purpose of facilitating treatment-related billing.


11. Indemnification

You agree to indemnify, defend, and hold harmless Arizona Psychiatry, its shareholders, partners, affiliates, directors, officers, employees, agents, suppliers, licensors, and any other parties involved in the operation and delivery of the Online Services, from and against any claims, liabilities, damages, losses, or expenses (including reasonable attorneys’ fees) arising from your use of the Online Services or your violation of these Terms and Conditions.


12. Limitation of Liability & Disclaimer of Warranties

    The Online Services and all content, materials, and information provided are offered on an “as is” and “as available” basis without any warranties, either expressed or implied. 16. Enforcement and Termination

Although patients do not create accounts on the Online Services, we reserve the right to suspend or terminate access to our Online Services if we suspect any violation of these Terms and Conditions. We may also take any other lawful action as we deem appropriate in response to any actual or suspected misuse of the Online Services.


17. Data Charges

If your access to the Online Services requires the use of wireless, cellular data, or internet access provided by third parties, you are solely responsible for any charges or fees incurred.


18. Miscellaneous

  • No Right to Use Content:
