Appointments

At Arizona Psychiatry, we strive to make the appointment process as smooth as possible. To get started, prospective patients must complete our secure HIPAA-compliant intake form, which takes about 20-30 minutes to complete. This form is comprehensive and helps us determine whether our office is the right fit for your treatment needs. Please take your time and provide thoughtful responses, as they allow us to understand your mental health concerns. We are adult psychiatrists and can only see patients ages 17 and older. Our practice specializes in medication management, so if you are only seeking therapy, you will need to find a dedicated therapist. Additionally, we do not perform fitness for duty evaluations, forensic or court-related evaluations, disability evaluations, or accommodation and emotional support animal requests. We also do not provide clearance to return to work for mental health reasons. Regarding insurance, we are NOT in network with AHCCCS, Medicare, Tricare, Workers’ Comp, or EAP programs, and we are out of network for most Medicare Advantage plans. The only Healthcare Marketplace plan we accept is AmBetter; we are out of network for all other Marketplace plans.
